Visas for Medical Treatment

What Do I Need to Apply for a Visa for Medical Treatment?

To have your medical condition taken into consideration when applying for a non-immigrant visa you will need to have the following information in addition to the standard documentation required for a non-immigrant visa as listed under How To Apply For a Visa at US Embassy Tanzania.

1. A letter, in English, from the applicant's local physician:

  • Providing the diagnosis of the applicant's condition; and
  • Explaining the nature of the ailment and the reason the applicant requires treatment in the United States.

2. Evidence of adequate financial resources.

  • Proof of financial support or Affidavit of Support (Form I-134) from sponsor. Notarized originals only - faxes and photocopies are not acceptable.
  • Sponsor's assurance that patient will be accompanied on the return trip to Tanzania if necessary.
  • Letter from sponsor's employer on company letterhead stating position, salary, full or part-time and duration of employment.
  • Letter from sponsor's bank on bank letterhead and signed by a bank officer.
  • Sponsor's previous year's income tax return.

3. A personal letter from the sponsor to the U.S. Embassy:

  • Providing the address in the U.S. where the patient will physically reside while undergoing treatment and/or follow-up check ups; and
  • Explaining the sponsor's reasons for wanting to sponsor the applicant (full history of the relationship, how and when it began, and exactly what is expected in return).

4. A letter from a U.S. physician explaining:

  • Patient's condition and willingness to treat
  • Estimated length of treatment (minimum and maximum)
  • Estimated cost of treatment (minimum and maximum estimates of doctors' fees, hospitalization fees, tests, medicines, and all medical-related expenses)
  • Assurances that the cost of the anticipated treatment will be paid and funds are readily available to pay the cost of treatment.
  • Specific appointment date(s)
